If your doctor has sent us a referral, you will receive a phone call from our office assistant to confirm that you want to pursue therapy. After verifying your insurance benefits, we will select the best therapist to fit your needs and they will call you to schedule.
If you do not have a referral from a doctor, we may need to request a referral depending on your insurance. Some insurance companies will allow an initial evaluation without a referral. You can schedule by calling, emailing, or filling out the Contact Form below.

If you did not complete the new client paperwork ahead of time online, please arrive early to your first appointment to fill out the paperwork in the office. Look for a clipboard at the front desk.
Please bring a photo ID and your insurance card(s) for us to copy, along with your medication list and medical history if needed.
If you’re coming for a feeding or swallowing evaluation, you may want to bring in specific foods/drinks for which you or your child are having difficulty.
If you wear glasses, hearing aids, and/or dentures, please bring and wear them. If you’re coming for a vocal cord dysfunction evaluation and your symptoms are induced by exercise, you can bring or wear athletic clothes for an exercise challenge. Please bring an inhaler if you use one.

We have two designated ADA parking spots and a ramp to the building. We do not have a button to automatically open the doors. Please come with someone who can assist you or contact us once you arrive, so we can open the doors for you.

We bill insurance and are in network with Aetna, First Choice Healthcare, Medicare, Medicaid, Moda, Oregon Health Plan (OHP), PacificSource Community Solutions, PacificSource Health Plans, PacificSource Medicare, Providence, Regence Blue Cross Blue Shield, and Triwest. We also accept auto and workman’s compensation insurance. We strongly encourage you to contact your insurance carrier to verify we are in-network, your benefits, and financial responsibility.
If you do not see your insurance plan listed, we can check if they will offer out-of-network coverage.
Ultimately, the financial responsibility for an evaluation or treatment remains with the client. A verification of benefits is not a guarantee of payment.
